What is it?: If you are an existing patient you can use this form to reorder a prescription for your medication.
Notes: Once you’ve filled in the form, the Patient Care Team and your Harpal Clinic doctor will review it. If you are due a follow-up, you may need to attend this before your prescription can be renewed.
